Outpatient procedure within past 30 days of BSI
Outpatient procedures include invasive and non-invasive gastrointestinal and genitourinary procedures.
No β-lactam or fluoroquinolone courses within past 90 days
1 β-lactam or fluoroquinolone course within past 90 days
≥ 2 β-lactam or fluoroquinolone courses within past 90 days
Documented colonization with ESBLs within past 12 months
