Diabetic foot infection

Empiric Antimicrobial Therapy

Mild to moderate, without recent antibiotic treatment

PEDIS grade of diabetic foot infection (IDSA infection severity)

  • Grade 2 (mild): local infection involving only the skin and subcutaneous tissue, with erythema 0.5-2 cm
  • Grade 3 (moderate): local infection with erythema >2 cm, or involving structures deeper than skin and subcutaneous tissue, and no systemic inflammatory response syndrome (SIRS)

Moderate with recent antibiotic treatment or severe infection

Grade 4 (severe): local infection with ≥2 of the following SIRS criteria:

  • BT >38°C or <36°C
  • HR >90/min
  • RR >20/min or PaCO2 <32 mmHg
  • WBC >12,000 or 4,000 μL or ≥10% immature forms
