Usual dose 2-8 g/d IV (2-4 divided doses)

Urinary tract infection 1 g IV q12h

40-80 mg/kg/d IV #3-4

(Increased to 160 mg/kg/d for severe intractable infections such as sepsis and meningitis)

No dose adjustment

CrCl <2075% recommended dose IV q6-8h

General Information

  • Gonorrhea

  • Pharyngitis - Sinusitis

  • Postoperative infection; Prophylaxis

  • Respiratory tract infection

  • Soft tissue infection

  • Urinary tract infectious disease

Perform renal function tests, such as BUN and serum creatinine.

Monitor blood cell count, and repeat culture and sensitivities if necessary.

Monitor decrease in temperature; signs of severe allergic reaction.

  • Neutropenia

  • Hypoprothrombinemia

  • Hepatotoxicity

  • Urticaria

  • Hypersensitivity reaction
