Empiric Treatment

PLUS

Mupirocin topical

Severe Beta-Lactam Allergy

  • Age <12: q8h
  • Age ≥12: q12h

Max: 600mg

≥ 2 serotonergic agents: Monitor for signs of serotonin syndrome and consider switching to another option

De-escalation and Duration

De-escalate to narrowest spectrum PO antibiotic based on susceptibilities

Duration: 7 days
