Daptomycin Renal Dosing

Not On Dialysis

Non-invasive gram-positive infections (e.g., skin and soft tissue infections):

  • 4-6 mg/kg IV q24h

Bacteremia and invasive infections:

  • Staphylococci (including methicillin-resistant) and enterococci: 6-10 mg/kg IV q24h
  • Vancomycin-resistant enterococci (VRE): 8-10 mg/kg IV q24h; 12 mg/kg IV q24h may be required for highly resistant strains

Endocarditis:

  • Methicillin-resistant staphylococci: 8-10 mg/kg IV q24h
  • Resistant enterococci (including VRE): 10-12 mg/kg IV q24h

Intermittent Hemodialysis

Non-invasive infections and bacteremia:

  • 6 mg/kg IV after dialysis 3 times weekly

MRSA or VRE bacteremia, invasive infections and endocarditis:

  • 8 to 10 mg/kg IV after dialysis 3 times weekly; 12 mg/kg IV after dialysis 3 times weekly may be required for highly resistant strains

CVVHDF

6 mg/kg IV q24h

Peritoneal Dialysis

Non-invasive gram-positive infections (e.g., skin and soft tissue infections):

  • 4-6 mg/kg IV q48h

Bacteremia and invasive infections:

  • Staphylococci (including methicillin-resistant) and enterococci: 6-10 mg/kg IV q48h
  • Vancomycin-resistant enterococci (VRE): 8-10 mg/kg IV q48h; 12 mg/kg IV q48h may be required for highly resistant strains
