S. pyogenes (Group A strep)
Penicillin V 8.5 to 16.5 mg/kg PO TID
OR
Amoxicillin 8.5 to 16.5 mg/kg PO TID
Clindamycin 7 to 13 mg/kg PO TID
Clarithromycin 7.5 mg/kg PO BID
Avoid all beta-lactam antibiotics if severe delayed reaction to beta-lactams

Severe Delayed Reactions
Stevens-johnson syndrome
Toxic epidermal necrolysis
Drug reaction with eosinophilia and systemic symptoms
