Dosing

For outpatient cefazolin (when CADD pump not available) CrCl 30 - 50 mL/min: CrCl 10 - 30 mL/min: CrCl < 10 mL/min:1 g PO 30 min before each cefazolin dose- Cefazolin 2 g IV q24h with probenecid 1 g PO 30 min before each cefazolin dose OR

  • Cefazolin 2 g IV q12h without probenecidCefazolin 2 g IV q12h without probenecidCefazolin 2 g IV q24h without probenecid

General Information

Used to prolong beta-lactam serum levels to facilitate outpatient use of intravenous antibiotics, usually cefazolin when CADD pump not available

  • Allergic reactions including anaphylaxis

  • May cause exacerbation of gout

  • Gastrointestinal upset, may administer with food to minimize GI effects.

Avoid concomitant use of probenecid and methotrexate if possible. Can increase levels of methotrexate.
