In case of bone or joint involvement:

• Postamputation with no residual infected tissue: 2-5 days
• Residual infected soft tissue (but not bone): 1-3 weeks
• Residual infected (but viable) bone: 4-6 weeks (initial parenteral, then consider oral switch) 
• No surgery, or residual dead bone postoperatively: ≥ 3 months (initial parenteral, then consider oral switch)
