Sexually Transmitted Infection Screening and Treatment

Asymptomatic Screening/Treatment Algorithm for Non-Pregnant Adults

Screening Recommendations

  • Consult infectious disease with any screening or treatment questions or with concern for neurosyphilis
  • Screen pregnant women for syphilis three times during pregnancy:
    • At first prenatal visit
    • At beginning of 3rd trimester
    • At delivery
  • Screen high risk patients (MSM or many sexual partners) every 3-6 months
  • Consider penicillin desensitization for penicillin allergic pregnant patients and patients with neurosyphilis
  • Screen annually: sexually active women <25 yrs, women >25 years at increased risk of infection
  • Screen MSM at least annually, every 3-6 months depending on risk
  • Screen patients at all sites of activity (urine, rectum, oropharynx)
  • Retest 3 months after treatment to identify reinfection, especially if sexual partners are not treated
  • Screen once: all patients aged 13-64
  • Screen MSM at least annually, every 3-6 months depending on risk
  • Screen all pregnant women at first prenatal visit
  • Retest pregnant women in third trimester if at high risk
