Monotherapy with beta-lactam is recommended for clinically stable patients
If suspect meningitis 2g IV q8h

Add only if suspect catheter related infection, pneumonia, skin infection, +MRSA colonization or mucositis
Unexplained Fever: Continue antibiotics until there are clear signs of marrow recovery; traditionally noted by ANC >500
Documented Infection: The duration is dictated by that infection and antibiotics should continue at least until marrow recovery
For clinically stable patients, consider adding anti-fungal coverage IF still febrile after 4-7 days on antibiotics
