Remove catheter if possible, or exchange catheter if in place for ≥ 2 weeks prior to collecting urine for culture
If asymptomatic with old foley, consider removing foley and repeating urine culture as catheter colonization is extremely common
Cefepime 1g IV q6h

Piperacillin-Tazobactam 4.5g IV q6h
