Ceftriaxone 1-2g IV daily
PLUS
Azithromycin 500mg IV/PO q24h x 3 days
OR
Doxycycline 100mg IV/PO q12h x 5 days
Ampicillin/Sulbactam 3g IV q6h
Levofloxacin 750mg IV/PO daily x 5 days
5 days of therapy for most patients is non-inferior to 7-14 days
7 days of therapy for moderate immunocompromised or SLD
10-14 days if poor clinical response or immunocompromised/SLD
