Preferred for use only in patients whose infections meet one of the following conditions:
- Infections due to gram-negative organism(s) and at least one of the following apply: Patient with confirmed allergy to both penicillin and cephalosporin treated for community-acquired pneumonia, intra-abdominal infection, bacteremia, or pyelonephritis
- Definitive therapy for multi-drug resistant organism in which the isolate is resistant to beta-lactams but susceptible to ciprofloxacin or levofloxacin
- For step-down therapy from intravenous to oral where fluoroquinolones are the only susceptible and appropriate agent available
- Traveler’s or infectious diarrhea (Shigella, Salmonella, Cholera)
- Prostatitis/epididymal orchitis
- Neutropenic fever bacterial prophylaxis or in combination with other anti-pseudomonal agent(s) for treatment
- Cystic fibrosis exacerbation
- Culture history or previous microbiological failure with preferred agents
- Serious Legionella infection
- Mycobacterial infections
- Typhoid/paratyphoid if anaphylaxis to ceftriaxone
