Criteria for Use

Preferred for use only in patients whose infections meet one of the following conditions:

  • Infections due to gram-negative organism(s) and at least one of the following apply:
    • Patient with confirmed allergy to both penicillin and cephalosporin treated for any of the following: community-acquired pneumonia, intra-abdominal infection, bacteremia, pyelonephritis
    • Definitive therapy for multi-drug resistant organism in which the isolate is resistant to beta-lactams but susceptible to ciprofloxacin or levofloxacin
    • For step-down therapy from intravenous to oral where fluoroquinolones are the only susceptible and appropriate agent available
    • Traveler’s or infectious diarrhea (Shigella, Salmonella)
    • Prostatitis/epididymal orchitis
    • Neutropenic fever bacterial prophylaxis or in combination with other anti-pseudomonal agent(s) for treatment
    • Cystic fibrosis exacerbation
    • Culture history or previous microbiological failure with preferred agents
  • Serious Legionella infection
  • Mycobacterial infections
  • Typhoid/paratyphoid if anaphylaxis to ceftriaxone
