Empiric Treatment

If patient is critically ill - consider addition of ciprofloxacin or aminoglycoside while awaiting blood culture results and susceptibilities.

For Severe Beta Lactam Allergy

OR 500-750mg PO BID

If patient is critically ill - consider addition of aminoglycoside while awaiting blood culture results and susceptibilities.

Treatment Notes

Gram-negative infective endocarditis is extremely rare. Routine echocardiography is not advised unless there is high clinical suspicion of endocarditis.

If patient has risk factors for ESBL infection - consider initiation of carbapenem (e.g. Meropenem) based regimen.
