S. aureus is the most common pathogen

Gram negatives more likely if recent antibiotic use or chronic ulcer

Anaerobes

Particularly in deeper wounds, more chronic, or accompanied by necrosis or ischemia.

  • Uncommon pathogen in DFI
  • Primary risk factor is exposure to water (e.g. hot tub, soaking)
  • Isolation can reflect colonization of an open wound/ulcer
  • Expanding coverage to include Pseudomonas when found on culture only necessary if patient is failing empiric regimen
