Clinical Considerations

Antibiotics should be administered <60 minutes before incision (<120 minutes for Vancomycin)

If antibiotics are ordered post-operatively, the last dose should be given < 24 hours from surgery end

Limiting the duration of antimicrobial prophylaxis as a single dose preoperative dose can reduce the risk of Clostridium difficile disease

  • May cause hypotension, which could be especially dangerous during induction of anesthesia
  • Even when the drug is given over 60 minutes, hypotension may occur; treatment with diphenhydramine or further slowing of rate may help
  • In patients weighing more than 75 kilograms, some experts would give 15 milligrams/kilogram to a maximum of 1.5 grams/dose at a slower infusion rate (over 90 minutes for 1.5 grams)
