Clinical Consideration

  • Chronic heart, lung, liver, or renal disease
  • Diabetes
  • Alcoholism
  • Malignancy
  • Asplenia
  • Tobacco use

Severe Beta-Lactam Allergy

Duration of Therapy

5 days. Extension of therapy beyond 5 days should be guided by validated measures of clinical stability, which include resolution of vital sign abnormalities, ability to eat, and normal mentation.

De-escalation

If Respiratory Panel rules out Chlamydia, Pertussis, and Mycoplasma, and suspicion for Legionella is low, azithromycin may be discontinued
