Clinical Considerations

Oral acyclovir is recommended for those presenting within 72h of symptoms, or longer if lesions are still appearing

Benefits are greatest in those >50 years, but treatment can be considered for younger patients

PHN is diagnosed when localized neuropathic pain persists beyond three months in the same dermatome(s) affected by acute herpes zoster

Risk factors for PHN are:

  • Age >60 years
  • Severe pain with acute herpes zoster
  • Extensive rash with acute herpes zoster

Consider neuropathic pain control as appropriate

  • All immunocompromised patients should be treated immediately, even if after 72h
    • IV treatment may be necessary
  • Discuss with Infectious Diseases
