Clinical Considerations

Infectious agents in AECOPD can be viral or bacterial

  • Treatment of AECOPD with antimicrobials is controversial

  • GOLD criteria can help to determine when to use empiric antibiotics

Cardinal Symptoms:

  • ↑ Dyspnea

  • ↑ Sputum volume

  • ↑ Sputum purulence

Consider antibiotics if patient:

  • has 3 cardinal symptoms

  • has 2 symptoms with one being ↑ sputum purulence

  • requires mechanical ventilation

Consider patient antibiotic exposure in previous 3 months

Antibiotic dosing assumes normal renal function
