Dosing

General Information

  • Targeted therapy of resistant gram positive infections with MRSA (particularly when MIC≥2) and VRE including endovascular infection.

Limitations of use: CANNOT be used in pneumonia, as it is inactivated by lung surfactant in alveoli.

  • CK should be monitored at least weekly (monitor for muscle pain or weakness)
  • CBC, renal, hepatic function
  • Diarrhea
  • Headache, dizziness
  • CK elevation, myopathy, rhabdomyolysis
  • Increased INR
  • Peripheral neuropathy
  • LFT elevations
  • Renal impairment
  • Eosinophilic pneumonia
  • Rash

Statins - holding statin is recommended during daptomycin therapy, due to the potential for CK elevation, myopathy.

Higher doses used for endovascular infection.

Inactivated by pulmonary surfactant and therefore insufficient for pulmonary infection.

Restricted to Infectious Diseases (ID)/Antimicrobial Stewardship Program (ASP) within 48 hours of being dispensed.
