Possible Pathogens


Blood cultures positive in most cases of Candida

Aspergillus (less common)

  • Blood cultures rarely positive with Aspergillus
  • Culture negative IE, usually in patient with prosthetic cardiac valve

Noncardiac sites of metastatic infection may occur

Ie. endophthalmitis - requiring systemic and intraocular antifungal

Recommended Treatment

Surgical replacement of infected valve



Once completion of initial therapy, long-term (lifelong) suppressive therapy with an oral azole is reasonable, especially in those deemed inappropriate for surgery
