Gram positive cocci in short chains
Nosocomial (esp. VRE), associated with prior antibiotics
Endocarditis (usually subacute)
Intra-abdominal or hepatobiliary infection
More likely to cause true infection in immunocompromised individuals or after selection pressure from antimicrobial administration has eliminated other organisms.
E. faecium are more likely than E. faecalis to be vancomycin resistant.
