Microbiology

Empiric Treatment

Fluid resuscitation as needed

Percutaneous drainage of abscesses preferred over surgical drainage

Risk Factors for Failure of Source Control:

  • Delay in initial intervention (>24hrs)
  • High illness severity (APACHE II ≥15)
  • Advanced age
  • Comorbidity and degree of organ dysfunction
  • Low albumin level
  • Poor nutritional status
  • Degree of peritoneal involvement / diffuse peritonitis
  • Inability to achieve adequate debridement / control of drainage
  • Malignancy

Initiate as soon as diagnosis of IAI is made or highly suspected
