Community Acquired Pneumonia

Treatment

Do NOT add anti-anaerobic therapy for suspected aspiration pneumonia unless lung abscess or empyema is suspected.

  • Beta-lactams, FQ, tetracyclines: 5 days

  • Azithromycin: 1500 mg total

    • Azithromycin 500 mg q24hr x 3 doses has been shown to be equivalent to azithromycin 500 mg x 1 day followed by 250 mg q24hr x 4 days

Risk Factors

  • History of MRSA in previous 12 months, OR
  • Hospitalization with IV antibiotics in previous 90 days AND at least one of the following:
    • Post-influenza pneumonia
    • Structural lung disease
    • Empyema or lung abscess
    • IV drug use
    • Incarceration
  • History of Pseudomonas aeruginosa in previous 12 months, OR
  • Hospitalization with IV antibiotics in previous 90 days AND at least one of the following:
    • COPD
    • Bronchiectasis
    • Structural lung disease
