Dosing

General Information

Suspected or proven MRSA, coagulase-negative Staphylococcal infections, Enterococcal infections.

Goal AUC/MIC value is 400-600.  

AUC/MIC > 650 associated with higher risk of nephrotoxicity.

Trough levels drawn 30 minutes prior to infusion.

  • Nephrotoxicity

  • Cytopenias

  • Rash including Stevens-Johnson Syndrome

  • Red man syndrome (histamine release- slow down infusion)

Aminoglycosides may potentiate nephrotoxicity.

May enhance neuromuscular blockade of NM blocking agents.

Careful with concomitant nephrotoxins.

Medication cost calculated per day (24 hours) of therapy for the average adult dose (70kg) of 1g IV q12h.
