Dosing Infants & Children

9 mg/kg/dose IV q24h

  • Dose limit: 800 mg/dose prior to TDM Once-daily dosing is the preferred dosing regimen at SickKids.

Exclusion Criteria:

  • <5kg
  • Renal failure -acute kidney injury (serum creatinine above 1.5 x baseline or upper limit of age-specific normal range), chronic renal failure, or dialysis.
  • Patients for whom other dosing regimens below apply.

2.5 mg/kg/dose IV/IM q8h

  • Dose limit: 120 mg/dose prior to TDM
  • See renal dosing, if renal impairment.

1 mo to <6 yrs:

  • 10.5 mg/kg/dose IV q24h

≥6 yrs:

  • Females: 9.5 mg/kg/dose IV q24h
  • Males: 7.5 mg/kg/dose IV q24h
  • Dose limit: 800 mg/dose prior to TDM
  • ≤5 kg:
    • 2.5 mg/kg/dose IV q12h
  • >5 kg:
    • 9 mg/kg/dose IV q24h
    • Dose limit: 800 mg/dose prior to TDM
  • See renal dosing, if renal impairment.

2.5 mg/kg/dose IV q12h for 72 hours

  • Dose limit: 120 mg/dose

2.5 mg/kg/dose IV pre-closure and q12h x 1 dose post-closure

  • Dose limit: 120 mg/dose

Use Tobramycin, unless contraindicated

5 mg/kg PO TID

  • Use IV formulation for PO
