Routine Precautions

Urgent line removal indicated in CVL tunnel infections and uncontrolled sepsis.

Common Pathogens

S. aureus

Gram negative enteric bacilli

Treatment

Vancomycin

+/-

Gram negative coverage

Add gram negative coverage as indicated in immunocompromised patients/hospital-acquired infection

Duration of Therapy

*Uncomplicated: Cultures become negative and fever resolves within 72 hours in a patient without intravascular hardware, no endocarditis, without immunocompromise

Gram negative bacilli: 7-14 days from 1st negative culture, if the catheter is removed

Candida spp: 14 days from 1st negative culture, catheter must be removed

Staph aureus: At least 14 days from 1st negative culture, catheter must be removed
