Documented or Suspected Candida Infection
3 mg/kg/dose IV once daily
Dose limit: 300mg/dose, or 500mg/dose for severe infections
Documented or Suspected Aspergillus Infection
5mg/kg/dose IV once daily
Dose limit: 500mg/dose for severe infections
Higher doses of 7.5 to 10 mg/kg/dose may be used in consultation with Infectious Disease service.
Empiric Therapy in Febrile Neutropenia
3 mg/kg/dose IV once daily
Dose limit: 500mg/dose for severe infections
Dosing in Obesity
In obese patients (TBW at least 120% of ideal body weight) at least 1 year old, dose based on adjusted body weight or dose limit of 300 mg, whichever is lower. Total body weight is recommended in severe infections.
