5 to < 40 kg

5 mg/kg IV x 1, then 2.5 mg/kg IV q24h

Duration: up to 5 days for patients not requiring mechanical ventilation, may extend up to 10 days if no clinical improvement.

40 kg or greater

200 mg IV x 1, then 100 mg IV q24h

Duration: up to 5 days for patients not requiring mechanical ventilation, may extend up to 10 days if no clinical improvement.
