Gentamicin

Neonatal

Dosing by Post-Natal Age

0-7 days post-natal age

<34 weeks post-menstrual age (PMA): 3 mg/kg/dose IV q24h
≥34 weeks: 3 mg/kg/dose IV q18h

>7 days

≤1 kg: 3.5 mg/kg/dose IV q24h
>1 kg and <37 weeks PMA: 2.5 mg/kg/dose IV q12h
>1 kg and ≥37 weeks PMA: 2.5 mg/kg/dose IV q8h

Neonatal Cardiac Patient Dosing

CCCU

>7 days post natal age: 2.5 mg/kg/dose IV q12h

Open Sternum Prophylaxis

3.5 mg/kg/dose IV q24h for 72 hours

Sternal Closure Prophylaxis

3.5 mg/kg/dose IV pre-closure only

Intestinal bacterial overgrowth

Use Tobramycin, unless contraindicated

5 mg/kg/dose PO tid

No TDM required for oral use Use IV formulation for PO Use for 14 days then cycle to oral metronidazole for 14 days

Synergy for Group B Strept Meningitis or Sepsis

Use dosing as for Newborn Infants by Age, Weight, and PMA
