General Information

Synergy with beta-lactams for gram positive infections, in combination with other active agents for the treatment of susceptible pathogens

Telephone EAP Criteria:

  • Dosage Form: 40 mg/mL injection
  • Reimbursement Criteria: For treatment of infections susceptible to gentamicin
  • Standard Approval Duration: As requested up to 5 years
  • Nephrotoxicity (non-oliguric) - less common with once daily dosing; greater toxicity with longer duration and supratherapeutic trough levels; avoid concomitant nephrotoxins
  • Vestibulocochlear toxicity (irreversible) - require audiology testing if prolonged use
  • Can exacerbate neuromuscular blockade - e.g. contraindicated in patients with myasthenia gravis
  • Calculate dose according to adjusted body weight
    • Adjusted Body Weight (ABW)(kg) = IBW + 0.4 (actual body weight - IBW)
