Given as part of a multi-drug regimen

• 15 mg/kg/dose PO once daily (higher doses of up to 25 mg/kg/day may be used in consultation with the Infectious Diseases service)

OR

• 50 mg/kg/dose PO twice weekly given as DOT (directly observed therapy) 

Dose limit: 2.5 g/dose
