General Dosing

  • 8 mg/kg/dose PO once daily

OR

  • 4 mg/kg/dose PO q12h

Dose limit: 400 mg/dose

High Dose

Higher doses of 10-15 mg/kg/dose PO q12h have been used in the treatment of serious infections (eg, typhoid fever) or as step down from IV therapy

Gonococcal Infections (Alternative option due to increasing rates of resistance in Ontario)

< 9 years old or ≤45 kg:

  • 8mg/kg PO BID x 2 doses
  • Dose limit: 400mg/dose

≥9 years old or >45 kg:

  • 800mg PO once

In combination with azithromycin
