Cystic Fibrosis (for non-lung transplant only)

25 mg/kg/dose IV q24h
Dose limit: 1500 mg/day prior to TDM

or 

50mg/dose Inhaled daily or BID
Dose limit: 1000 mg/day

Haematology/Oncology & HPCT Patients with Fever and Neutropenia

• 2 months to <11 yrs: 35 mg/kg/dose IV q24h

• 11 to <16 yrs: 25 mg/kg/dose IV q24h

• ≥16 yrs: 20 mg/kg/dose IV q24h
