Open Sternum Prophylaxis
- 0-7 days; 30 mg/kg/dose IV q12h x 72h
- >7 days; 30 mg/kg/dose IV q8h x 72h
Sternal Closure Prophylaxis
- 0-7 days; 30 mg/kg/dose IV pre-closure and q12h x 1 dose post-closure
- >7 days; 30 mg/kg/dose IV pre-closure and q8h x 2 doses post-closure
