10 mg/kg/dose PO once on day 1 (dose limit: 500 mg/dose) followed by 5 mg/kg/dose PO once daily on days 2 to 5 (dose limit: 250 mg/dose)

Acute Otitis Media

Single dose regimen

30 mg/kg/dose PO as a single dose, dose limit: 1500 mg/dose

3 day regimen

10 mg/kg/dose PO once daily for 3 days, dose limit: 500 mg/dose

5 day regimen

10 mg/kg/dose PO on day 1 (dose limit: 500 mg/dose), followed by 5 mg/kg/dose (dose limit: 250 mg/dose) PO once daily on days 2 to 5

Cystic Fibrosis, as An Anti-Inflammatory Agent

18 to 36 kg

250 mg 3 times weekly (eg, Monday, Wednesday, Friday)

≥36 kg

500 mg 3 times weekly (eg, Monday, Wednesday, Friday)

Mycobacterium Avium Complex

10 to 12 mg/kg/dose PO once daily in combination with another antimycobacterial agent Dose limit: 500 mg/dose

Group A Streptococci (GAS) Pharyngitis

12 mg/kg/dose PO for 5 days Dose limit: 500 mg/dose

Pertussis, Treatment and Postexposure Prophylaxis

1 month to 6 months

10 mg/kg/dose PO q24h

>6 months

10 mg/kg once on Day 1 (dose limit: 500 mg/dose), followed by 5 mg/kg once daily on Days 2-5 (dose limit: 250 mg/dose)

Usual duration of therapy is 5 days

Chlamydial Urethritis, Cervicitis and Pelvic Inflammatory Disease
