Restricted Indications
Oral therapy for infections (eg, Pseudomonas, Enterobacter) that are not susceptible to other oral agents
Infections caused by Gram negative organisms which are not susceptible to other agents
Infections caused by Gram negative organisms in patients who cannot tolerate other agents
Infections in patients with severe Beta-lactam allergy
Chest exacerbation in cystic fibrosis (PO, IV)
Suspected or proven Mycoplasma in patients intolerant to oral macrolide. Levofloxacin IV is preferred for this indication
Haematology/Oncology & HPCT patients with fever and Beta-lactam allergy
Other Indications
Infectious Diseases approval required
Use levofloxacin for IV Mycoplasma coverage, and improved S. pneumonia coverage compared to ciprofloxacin
