General Dosing

Mild to moderate infections

5-7.5 mg/kg/dose PO/IV q6h or 7-10 mg/kg/dose PO/IV q8h Dose limit: 1.8 g/day PO; 2.7 g/day IV

Severe infections

7.5-10 mg/kg/dose IV q6h or 10-13 mg/kg/dose IV q8h Dose limit: 3.6 g/day IV

Use enteral route when possible due to excellent bioavailability, decreased cost, and reduced side effects (e.g. thrombophlebitis)

Chloroquine-resistant falciparum malaria

IV therapy

10mg/kg/dose IV x1, followed by 5 mg/kg/dose IV q8h for 7 days

PO therapy

5 mg/kg/dose PO q6h for 7 days OR 6.7 mg/kg/dose PO q8h for 7 days Dose limit: 2.7 g/day

Use with quinine for chloroquine-resistant malaria only if patient is unable to take doxycycline or tetracycline.

Piperacillin-allergic neutropenic patient with fever

in combination with gentamicin and ciprofloxacin

10mg/kg/dose IV q8h Dose limit: 600mg/dose IV
