Disseminated HSV infection

Severe mucocutaneous HSV infection

< 12 years: 10mg/kg/dose IV q8h ≥ 12 years: 5-10mg/kg/dose IV q8h

PO dosing (following IV therapy)

20mg/kg/dose PO q6h Dose limit: 1.6g/day (400mg PO q6h)

Varicella or Zoster Treatment in Immunocompromised hosts

<1 year

17-20mg/kg/dose IV q8h

≥1 year

500 mg/m2/dose IV q8h

PO dosing (following IV therapy)

20mg/kg/dose PO q6h x 5 days

Mild-Moderate mucocutaneous HSV infections

10-20mg/kg/dose PO q6h Dose limit: 800 mg/day PO (200 mg PO q6h)

HSV prophylaxis in Immunocompromised hosts

13 mg/kg/dose PO q8h or 20 mg/kg/dose PO q12h

HSV prophylaxis in HSCT recipients

< 40kg

250 mg/m2/dose IV q8h

≥ 40 kg

250 mg/m2/dose IV q12h

Lung Transplant Prophylaxis

No EBV risk

≤12 years: 250 mg/m2 PO q8h x 3 months Dose limit: 600mg/day (200mg/dose)

Varicella in an immunocompetent host

20 mg/kg/dose PO q6h x 5 days

Dose Limit: 3.2 g/day PO (800 mg PO q6h)

Intestinal Transplant Patients

HSV positive without CMV/EBV risk

250 mg/m2 PO q8h x 12 weeks Dose Limit: 600 mg/day (200 mg/dose)
