Usual Dose

Target Trough of 5-12mg/L

10 mg/kg/dose IV q6h Dose limit: 500 mg/dose prior to TDM

Severe infections including meningitis, unstable neutropenic patients with fever, sickle cell disease, MRSA infections

Target Trough of 10-15mg/L

15 mg/kg/dose IV q6h Dose limit: 1 g/dose prior to TDM

Cardiac Surgery

15 mg/kg/dose IV q12h Dose limit: 1 g/dose prior to TDM

Cardiac Sugery Prophylaxis MRSA +

15mg/kg/dose IV q12h x 48 hours

C. difficile colitis

10mg/kg/dose PO/NG q6h

Dose limit:

  • Uncomplicated infection 125mg/dose (500mg/day) PO/NG
  • Complicated/fulminant infection 500mg/dose (2000mg/day) PO/NG
