General Information

IV: Red man syndrome (histamine release - slow down infusion rate), nephrotoxicity, cytopenias, rash including Stevens-Johnson Syndrome

  • Trough: 0-30 min before dose
  • Draw pre-dialysis concentration, preferably 15-30 minutes before start of dialysis
    • CNS infections: 10-15 mg/L
    • Other infections: 5-12 mg/L

Monitoring Parameter: Periodic renal function tests (especially when targeting higher serum concentrations), urinalysis, serum vancomycin concentrations, WBC; audiogram (in patients who concurrently receive ototoxic chemotherapy); fluid status

  • With or Without Food

  • Calculate doses according to adjusted body weight
  • The IV formulation may be used for oral dosing
