  • Weight 1.2-2kg: 7.5 mg/kg/dose IV/IM q24h
  • Weight >2kg: 7.5 mg/kg/dose IV/IM q12h
>7 days
  • Weight 1.2-2kg: 7.5 mg/kg/dose IV/IM q12h
  • Weight >2kg: 15 mg/kg/dose IV/IM q12h

Dose adjustment required in severe renal or hepatic impairment

Intestinal bacterial overgrowth

In infants with short bowel syndrome or carbohydrate intolerance

10 mg/kg/dose PO q8h x 14 days

Use IV formulation for PO
