General Dosing

Mild to Moderate Infections
  • 12.5-37.5 mg/kg/dose PO/IV/IM q6h
  • Usual adult dose: 500 mg/dose PO

Dose limit: 1 g/dose PO/IV/IM

Severe Infections
  • 25-35 mg/kg/dose IV/IM q4h
  • 37.5-50 mg/kg/dose IV/IM q6h (up to 300 mg/kg/day may be used in select cases in consultation with ID)

Dose limit: 12 g/day IV/IM (2 g/dose IV/IM)
