Meningitis in children ≥1 month of age

  • 100 mg/kg/dose IV at 0, 12, and 24h followed by 100 mg/kg/dose IV q24h

OR

  • 50 mg/kg/dose IV q12h

Dose limit: 2 g/dose, 4 g/day

Sepsis, severe pneumonia in ICU patients, sickle cell disease, osteomyelitis, and other severe infections

100 mg/kg/dose IV/IM q24h Dose limit: 2 g/day

Endocarditis

50 mg/kg/dose IV q12h

Dose limit: 2 g/dose; 4 g/day

Community-acquired pneumonia (non-severe), peritonitis, arthritis, bacteremia

50 mg/kg/dose IV/IM q24h Dose limit: 1 g/day

Appendicitis (perforated)

50mg/kg/dose IV q24h in combination with metronidazole Dose limit: 2g/day

Otitis Media

Initial Treatment

50mg/kg/dose IM daily x 1 to 3 days
Dose limit: 1 g/day

If symptoms persist after the first dose, additional doses are necessary. Patients should be followed up closely.

Treatment Failure

50mg/kg/dose IM daily x 3 days
Dose limit: 1 g/day

Uncomplicated Gonococcal Infection

In combination with azithromycin

50mg/kg IM Once Dose limit: 250mg/dose
