General Information

Urinary Tract Infection, especially involving ESBL producing organisms.

Offlabel - Prostatitis.

Restriction: Restricted to ID expert or designee

Criteria for Use:

  • Salvage therapy for UTI due to multi-drug resistant organisms (e.g. ESBL, VRE, CRE, +/- Pseudomonas with confirmed sensitivity to fosfomycin)
  • UTI in patients with multiple drug allergies or when no other PO option is available

Unacceptable Uses:

  • Asymptomatic bacteriuria
  • Infections outside of the urinary tract

Advisory: Susceptibility to fosfomycin should be confirmed prior to initiation of therapy

  • GI upset, especially diarrhea
  • Headache
  • Eosinophilia, neutropenia
  • Hypokalemia

No significant interactions
