Clindamycin 900 mg IV q8h
PLUS
Gentamicin 2 mg/kg/dose IV once STAT loading dose then, 1.8 mg/kg/dose IV q8h maintenance dose
Until patient improves and is afebrile for 24-48h. Oral step down is not indicated after IV therapy with appropriate resolution of symptoms
