General Information

  • Empiric (in combination) or targeted therapy for suspected or confirmed gram negative infections
  • Used synergistically in enterococcal endocarditis
  • Injection: 10 mg/mL, 40 mg/mL

Neonatal:

  • IV, IM: 10 mg/mL

Administer extended interval and traditional interval doses over 30 minutes

Antimicrobial class: Aminoglycoside

Average serum half life:

  • Neonates < 1 week: 3-11.5 hours
  • Neonates 1 week to 1 month: 3-6 hours
  • Infants: 4 ± 1 hour
  • Children: 2 ± 1 hour
  • Adolescents: 1.5 ± 1 hour
