Duration and De-escalation


  • Uncomplicated bacteremia (no metastatic complications): minimum 14 days from first negative culture
  • Complicated (e.g. endocarditis, osteomyelitis, unremovable source or bacteremia that persists more than 72 hours on appropriate therapy): requires 6 weeks of therapy

For all patients repeat blood cultures q48h until negative
