Surgical or endoscopic drainage to be considered if there is biliary obstruction, as antibiotics will not enter bile in the presence of obstruction
If high local prevalence of ESBL producing E. coli, Klebsiella sp. strains, start with Meropenem as first line antibiotic, and then de-escalated once antibiotic susceptibility is known
